These Terms and Conditions apply to Locum Referral Schemes run by Holt Doctors Limited or its sister company, The Anaesthetics Agency Limited. For the purposes of these Terms and Conditions Holt Doctors Limited and The Anaesthetics Agency Limited shall each be referred to as “us”, “we”, “our”, “the company” as the context so provides.
Refer a Friend Scheme – summary
The Recommender – A doctor who refers a fellow doctor to us for work.
The Recommended Doctor – A doctor who is recommended to us by the Recommender through this scheme.
Recommendation Scheme
Doctors who recommend a colleague (ST3-ST8) who goes on to work 30 hours within 12 months of registration with us are eligible for a reward to the value listed in the table below, paid in either:
- Lifestyle Vouchers
- Amazon vouchers
The breakdown of grades and specialties are as following:
Locum Doctor - £250
Consultant Doctor - £350
Where the criteria is met, the reward shall be paid to the Recommender in the amount set out in the table above and either as Lifestyle vouchers fromMotivates Inc.Ltd. www.thelifestylevoucher.co.uk or as Amazon vouchers fromAmazon.com, Inc - www.amazon.co.uk

Locum Recommendation Scheme – Terms and Conditions
Eligibility criteria - How to make a referral
1. You must register via the Company registration form or have been referred via the ‘Refer a friend’ form to be eligible.
2. We are unable to accept registrations or referrals which are not submitted through the forms noted in point 1.
3. This form is automatically date stamped with the date of submission.
4. It is the responsibility of the Recommender to:
a) obtain the Recommended Doctors’ consent before providing the Recommended Doctor’s contact details to the Recommendation Scheme; and,
b) advise the Recommended Doctor of where to find our privacy policy (found here).
5. The Recommender must provide clear details of the Recommended Doctor at the point of referral. HDL must be able to identify the Recommended Doctor from the details the Recommender provides.

The Recommended Doctor must provide clear details of the Recommender at the point of registration. Holt Doctors must be able to identify the Recommender from the details the Recommended Doctor provides.
6. There is no limit to the number of doctors that can be recommended and thus no limit to the number of rewards which may be payable to a Recommender.
7. The Recommender must themselves be registered with us to be eligible under the Recommendation Scheme.
8. Doctors can only be recommended once, irrespective of whether they go on to fulfil the Recommendation Scheme condition for a reward.
9. Doctors already registered, referred through another source, or already working through us, cannot be recommended and will not qualify under the scheme.
10. If the same referral is made by two separate Recommenders and a reward becomes due under the terms of the scheme, payment will only be made to the Recommender who was first to submit the Recommended Doctor as per the date stamp of the submission.
11. The Recommended Doctor must be eligible to work in the UK.
Rewards
12. In the case where the Recommender wishes to receive either a Lifestyle e-vouchers or Amazon voucher the voucher will be distributed by email to the Recommender before the end of the following month after which the Recommended Doctor first invoices 30 hours of work.
13. Depending on which voucher you choose; the voucher will come directly from RLDL ([email protected]) or Amazon ([email protected].)and it is the responsibility of the Recommender to ensure they are able to receive the voucher (spam filters etc).
14. Depending on which voucher the Recommender chooses, they will need to register their voucher on either the Lifestyle website or the Amazon website. With the Lifestyle voucher, the Recommender can select their preferred retailer/s. Vouchers can be used in-store or online and codes are supplied by Motivates Inc. Ltd. When the Recommender redeems an Amazon.co.uk voucher to their account, the funds are stored in their Account and will automatically be applied to their next eligible order.
15. We reserve the right to offer an alternative reward of the same value at any time.
How and when to claim a reward
16. To receive the Recommendation Scheme reward, the Recommended Doctor simply needs to invoice 30 hours' work within 12 months of registering with us. If this requirement is met, the Recommender will qualify for the reward.
17. We will aim to notify the Recommender of their eligibility to claim a reward by email, no later than the last Friday of each month. Due to volume, we are unable to provide regular status updates of hours worked.
18. The Recommender must claim the reward within 6 months of being notified of their eligibility. Once claimed, the reward can be enjoyed with no further restrictions.
General
19. The Recommender will be expected to account for any tax and NI where applicable through their self-assessment tax returns.
20. Holt Doctors will provide the Recommended Doctor with the name of the Recommender if requested by the Recommended Doctor.
21. Holt Doctors reserves the right to end this Scheme at any time and will post notice to this effect on the company website one calendar month prior to the closure of the Scheme. Recommenders will be advised of any cessation of the Scheme if their Recommended Doctor is yet to fulfil the Recommendation Scheme conditions for reward one month prior to the closure of the Scheme.
22. We do not accept responsibility or liability for vouchers not successfully received due to technical fault, technical malfunction, computer hardware failure, satellite, network or server failure of any kind.
23. We accept no liability for lost or stolen vouchers, once the voucher is received by the Recommender.

Consultant - £1000
Middle grade - £750
Junior grade - £500
How and when to claim a reward
15. The Recommendation Scheme reward will become payable when the Recommended Doctor has completed 6 months following appointment.
16. We will aim to notify the Recommender of their eligibility to claim a reward by email by the last Friday of each month (due to volume, we are unable to provide regular status updates of time worked since appointment).
17. The Recommender must claim vouchers within 6 months of being notified that their Recommended Doctor has completed 6 months following their appointment.
General

Loyalty Appraisal Bonus – Terms and Conditions

Introduction
The Loyalty Appraisal Bonus is all about rewarding you for making Holt Doctors your locum agency of choice and regularly working through us.
Once you complete your first locum shift with Holt Doctors, you'll be automatically enrolled in the Loyalty Appraisal Bonus scheme. Accumulate the required qualifying hours within a 12-month period and, when you are due your annual appraisal, book this through one of our approved appraisers and we'll cover the costs. You must also be connected to Holt Doctors as your designated body at the time of booking.
The number of hours that you are required to work in a 12-month period to qualify (“Qualifying Hours”) under the scheme varies depending on your grade, as follows:
- Consultants & GPs - 500 hours
- All Other Grades - 800 hours
Each scheme period runs from the month in which your first locum shift was invoiced for a period of 12 months (“Scheme Period”). The Scheme Period will automatically restart at the end of each 12-month period. Your hours total will restart at zero at the start of each Scheme Period. The start date for your individual Scheme Period will remain the same, even if you take a break from working.
Your qualifying hours are ‘banked’ for a further 6 months following the end of your scheme date to give you time to claim your Loyalty Appraisal Bonus and enable you to book your appraisal when it is due.

Terms and Conditions
- These Terms and Conditions apply to the Loyalty Appraisal Bonus scheme run by Holt Doctors (“we”, “us”, “our”, “the company”). Please read the Introduction section above as the information contained in this section forms part of the Terms and Conditions.
- The Loyalty Appraisal Bonus (the “Scheme”) is available to locum doctors working through the company (“you”, “your”).
- To qualify for reimbursement of your appraisal by the company you must:
- have accrued the required number of Qualifying Hours stipulated for your grade within the relevant Scheme Period;
- use one of our pre-approved appraisers (if you have an appraisal with an appraiser that is not pre-approved by us then the cost of the appraisal will not be reimbursed);
- be connected to Holt Doctors as your designated body at the time of booking your appraisal.
- Doctors who meet the Qualifying Hours threshold for the Loyalty Appraisal Bonus within the Scheme Period can book their appraisal for up to 6 months following the end date of their scheme. This provides a sufficient timeframe to utilise the benefits earned.
- If you do not accrue the required number of Qualifying Hours in any Scheme Period, your accrued hours will reset to zero at the end of the relevant Scheme Period.
- If you haven't accrued the required number of Qualifying Hours before the end of your scheme period, the non-rollover policy will apply, and you will not be permitted to carry the hours over into any subsequent Scheme Period.
- Hours worked may not be redeemed or exchanged for any other payment or benefit.
- We reserve the right to vary the Loyalty Appraisal Bonus Scheme Terms and Conditions at any time and will post notice to this effect on the company website prior to the change taking effect.
- We reserve the right to end the Loyalty Appraisal Bonus Scheme Terms at any time and will post notice to this effect on the company website one calendar month prior to the closure of the Scheme. The company will not be liable to pay or reimburse you for any accrued hours (whether in full or in part) at the end of the Scheme.
